CMS releases annual evaluation report for Optimistic Phase I outcomes

In January 2016, CMS, which funds Optimistic, released their annual evaluation report for the initiative to reduce avoidable hospitalizations among nursing facility residents. The outcomes of Phase 1 are impressive, and Phase 2 will begin October 1, 2016.

Outcomes for 2014 are:

  • Improved potential avoidable hospitalizations by 29.3%
  • Reduced all-cause hospitalizations by 21.2%
  • Lowered per-resident expenditures on all-cause emergency department visits by 30.9% and all-cause hospitalizations by 18.9%
  • Congratulations to the Optimistic Clinical Staff for receiving Phase 2 Award.
  • Facility and provider recruitment for Phase 2 partition is April 2016 - June 1 2016.
