G/IJ/SSQC Citation Update

There were a total of 44 recertification surveys completed by the Indiana State Department of Health in the month of July. Of those, five (11.36%) were found to be deficiency free. Three facilities received three IJs and a total of three SSQCs. F309 was cited for failure to provide care/services for the highest well being. The facility failed to use an appropriate intervention(s) to address a resident behavior by allowing another resident to attempt to calm a physically combative resident resulting in one resident being physically apprehended by another resident. The PASSR 2 indicated the resident had a history of behaviors, however no behavior plan was in place. F225 was cited for failure to ensure staff immediately reported, to the administrator, an observation of sexual encounter between two cognitively impaired residents. In the same facility F226 was also cited for failure to develop abuse and neglect polices and ensure they were fully and consistently followed by staff. F323 was also cited in the same facility for failure to prevent accidents related to inadequate supervision of the resident(s). F281 was cited for failure to administer sliding scale insulin according to the physician order and failure to complete a 2 AM blood glucose monitoring as ordered by the physician. The omission contributed to the resident experiencing severe life threatening low blood sugar during the night.
