Residential Care Citations

The ISDH conducted twelve surveys in the month of December with five or 41.67 percent deficiency free. There was a total of 41 residential tag citations with 3 Offense tags and 38 deficiency tags cited. Offensive tags as follows:

Tag 0241 (1 time) – Health Services for failure to ensure administration of medications and the provision of nursing care was as ordered by the physician and supervised by a licensed nurse on the premises or on call as follows: Medication shall be administered by licensed nursing personnel or qualified medication aides.

Tag 0243 (1 time) – Health Services for failure to observe for effects of medications and to ensure documentation of undesirable effects were contained in the clinical record and/or documented immediate physician notification.

Tag 0406 (1 time) – Infection Control for failure to establish an infection control practice designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of diseases and infection.

The most frequently cited tag continues to be tag 0273 (6 times) – Food and Nutrition for failure to ensure all food preparation and serving areas (excluding areas in residents’ units) are maintained in accordance with state and local sanitation and safe food handling standards.

Tag 117 (5 times) – Personnel for failure to ensure sufficient number of staff, qualifications, and training in accordance with applicable state laws and rules to meet the 24 hour scheduled and non-scheduled needs of the residents and services provided. The number, qualification, and training of staff shall depend on skills required to provide for the specific needs of the residents. A minimum of one wake staff person, with current CPR and first aid certificates, must be on site always. If 50 or more residents of the facility regularly receive residential nursing services or administration of medication, or both, at least one nursing staff person shall be on site always. Residential facilities with over one 100 residents regularly receiving residential nursing services or administration of medication, or both shall have at least one additional nursing staff person awake and on duty at all times for every additional 50 residents. Personnel shall be assigned duties in which they are trained to perform and conform to the appropriate job description.

Other tags were cited as follows:
Tag 0298 (2 times) – Pharmaceutical Services for failure ensure a consultant pharmacist was employed or under contract and held responsible for specified duties such as reviewing the drug handling and storage practices of ordering, storing, administering, and disposing of drugs.

Tag 0300 (2 times) – Pharmaceutical Services for failure to ensure over-the -counter medications, prescription drugs, and biologicals used in the facility were labeled in accordance with currently accepted professional principles. Label must include appropriate accessory and cautionary instructions and expiration date.

Tag 0148 (2 times) – Sanitation and Safety Standards for failure to maintain buildings, grounds, and equipment in a clean condition, in good repair, and free of hazards that may adversely affect the health and welfare of the residents or public. The facility must maintain a written program for maintenance to ensure the continued upkeep of the facility. Health Services tag 0240 was cited 2 times for failure to ensure the personal care, and assistance with activates of daily living were provided based upon individual needs and preferences.
