May Residential Care Citations

The Indiana State Department of Health conducted seven surveys in May and there were no Residential Facilities found to be deficiency free. A total of seven Offense tags were cited and a total of fifty-five residential tags cited.

Five facilities received offensive tag 0241 – Health Offense for failure to ensure administration of medications and the provision of residential nursing care was as ordered by the resident’s physician and supervised by a licensed nurse on the premises or on call. Medications to be administered by licensed nursing personnel or qualified medication aides.

Two facilities received offensive tag 052 – Residents’ Rights for failure to ensure residents were free from sexual abuse; physical abuse; mental abuse; corporal punishment; neglect; and involuntary seclusion.

Residential tags were as follows:

Tag 0117 (6 times) – Personnel Deficiency for failure to ensure staff were sufficient in number, qualifications, and or training in accordance with applicable state laws and rules to meet the twenty-four (24) hour schedule and unscheduled needs of the residents and services provided. A minimum of 1 staff person, with current CPR and first aid certificates, shall be on site always.
Tag 0217 (4 times) – Evaluation Deficiency for failure to identify and document the services to be provided by the facility. Tag 0214 (3 times) – Evaluation for failure to ensure each individual resident needs were initiated prior to admission and updated at least semiannually and upon a known substantial change in the resident’s condition, or more often at the resident’s or facility’s request. A licensed nurse must evaluate the nursing needs of the resident.
Tag 0145 (2 times) – Sanitation for failure of the facility to maintain equipment and supplies in a safe and operational condition and in sufficient quality to meet the needs of the residents.
Tag 0154 (2 times) – Sanitation and Safety Standards for failure to keep kitchens, kitchen areas, common dining areas, equipment, and utensils clean, free from litter and rubbish, and maintained in good repair.
Tag 0187 (2 times) – Physical Plant Standards for failure to ensure hot water temperature for bathing and hand washing facilities were controlled by an automatic control valve. Water temperature at the point of use must be maintained between one hundred (100) degrees Fahrenheit and one hundred twenty (120) degrees Fahrenheit.
Tag 0240 (2 times) – Health Services for failure to ensure personal care, and assistance with activities of daily living were provided based upon the individual needs and preferences of the resident.
Tag 0246 (2 times) – Health Services for failure to ensure PRN medications were administered by a qualified medication aide with authorization by a licensed nurse or physician. The QMA must receive appropriate authorization for each administration of a PRN medication and documented within the medical record each time.
Tag 0414 (2 times) – Infection Control for failure to ensure staff washed hands after each direct resident contact for which hand washing is indicated by accepted professional practice. Individual facilities received a tag for 036,048,053, 0118, 0144, 0148, 0275, 0300, 0301,0302, and 0304.
