Residential Care Citation Update

The Indiana State Department of Health conducted a total of 12 surveys in the month of June and five (41.67%) providers were found to be deficiency-free. There were nine offense tags cited, 47 deficiency tags cited and a total of 56 residential tags cited.

June Offense Tags
Tag 0241 (6 times) – Health Services Offense for failure to ensure the administration of medications and the provision of residential nursing care shall be as ordered by the resident’s physician and shall be supervised by a licensed nurse on the premises or on call as follows: (1) Medication shall be administered by licensed nursing personnel or qualified medication aides.

Tag 0052 (2 times) – Residents’ Rights Offense for failure to ensure residents were free from sexual abuse, physical abuse, mental abuse, corporal punishment, neglect and involuntary seclusion.

Tag 0406 (1 time) – Infection Control Offense for failure to establish and maintain an infection control practice designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of diseases and infection.

June Deficiency Tags
Tag 0273 (11 times) – Food and Nutritional Services for failure to ensure all food preparation areas (excluding areas in residents’ units) are maintained in accordance with state and local sanitation and safe food handling standards.

Tag 0090 (4 times) – Administration and Management for failure to Inform the division within twenty-four hours of becoming aware of an unusual occurrence that directly threatens the welfare, safety, or health of a resident.

Tag 0117 (4 times) – Personnel for failure to ensure staff were sufficient in number, qualifications, and training in accordance with applicable state laws and rules to meet the twenty-four (24) hour scheduled and unscheduled needs of the residents and services provided. A minimum of one (1) awake staff person, with current CPR and first aid certificates, shall be on site always.

Tag 0214 (4 times) – Evaluation for failure to ensure an evaluation of the individual needs of each resident initiated prior to admission and updated at least semiannually and upon a known substantial change in the resident’s condition, or more often at the resident’s or facility’s request. A licensed nurse shall evaluate the nursing needs of the resident.

Tag 0217 (4times) – Evaluation for failure to ensure completion of an evaluation. The facility must use appropriately trained staff members to complete the mandatory evaluation and must identify and document services to be provided by the facility.

Tag 0045 (3 times) – Residents’ Rights for failure to notify the resident of a transfer or discharge and the reasons for the move, in writing, and in a language and manner that the resident understands. The provider must place a copy of the notice in the clinical record. The following individuals or agencies must be notified of the transfer/move:

• The resident
• A family member of the resident if known
• Legal Representative
• The local long-term ombudsman program
• The person or agency responsible for the resident’s placement, maintenance, and care in the facility.

Tag 0144 (2 times) - Sanitation and Safety Standard for failure to ensure the facility is clean, orderly, and in a state of good repair, both inside and out, and provide reasonable comfort for all residents.

Tag 0154 (2 times) – Sanitation and Safety Standards for failure to ensure all kitchens and kitchen areas, common dining areas, equipment, and utensils clean, free from litter and rubbish, and maintained in good repair.
