Residential Care Citations and Regulatory Update - August 2017

ISDH conducted 15 surveys in August. There was a total of 63 residential tags cited and of those five of them being offence tags. Offensive tags as follows:

Tag 0052 (3 times) – Residents’ Rights Offense for failure to ensure residents were free from abuse.

Tag 0241 (2 times) – Health Services Offense for ensure administration of medications and the provision of nursing care was as ordered by the physician and/or supervised by a licensed nurse on the premises or on call as follows: (1) Medications shall be administered by a licensed nursing personnel or qualified medication aides. 

Residential tags are as follows:

Tag 273 (14 times) – Food and Nutritional Services for failure to ensure all food preparation and serving areas (excluding areas in resident units) are maintained in accordance with state and local sanitation and safe food handling standards.

Tag 144 (7 times) – Sanitation and Safety Standards for failure to ensure the facility is clean, orderly, and in a state of good repair, both inside and out, and shall provide reasonable comfort for all residents.

Tag 117 (5 times) – Personnel Noncompliance for failure to ensure the facility had specific procedures written and implemented of prospective employees.  Appropriate inquires shall be made for prospective employees. The facility shall have a personnel policy that considers references and any convictions in accordance with IC 16-28-13-3.

Tag 90 (4 times) – Administration and management Deficiency for failure to ensure administrator full responsibility for the overall management of the facility.  Responsibility includes: (1) Informing the division within 24 hours of becoming aware of an unusual occurrence that directly threatens the welfare, safety, or health of a resident. Notice of unusual occurrence may be made in telephone, followed by a written report, r by written report only that is faxed or sent by electronic mail within the 24 hour time. 

Tag 0214 (3 times) – Evaluation Deficiency for failure to ensure an evaluation of the individual needs of each resident is initiated prior to admission and updated at least semiannually and upon a known substantial change in the resident condition, or more often at the request of the resident or facility.  A licensed nurse to evaluate the nursing needs of the resident.

Tag 0217 (3 times) – Evaluation Deficiency for failure to ensure completion of an evaluation by using appropriately trained staff and/or appropriate identification of services to be provided by the facility.   Documentation will include appropriate scope, frequency, need, preference and discussed, reviewed and revised as changes occur.  A licensed nurse must participate if administration of medications and/or nursing care are services provided. 

Tag 086 (2 times) – Administration and Management for failure to ensure compliance with all applicable laws.

Tag 0153 (2 times) – Sanitation and Safety Standards for failure to ensure safety precautions taken related to oxygen storage and administered.  Residents on oxygen will be instructed on oxygen safety measures concerning storage and administration of oxygen. 

Tag 0240 (2 times) – Health Services Deficiency for failure to ensure personal care, and assistance with activities of daily living are provided based upon individual needs and preference. 

Tag 414 (2 times) – Infection Control for failure to ensure staff wash their hands after each direct resident contact for which hand washing is indicated by accepted professional practice. 

Other tags cited (1 time) are as follows: 029, 036, 055, 0118, 0145, 0147, 0148, 0149, 0191, 0215, 0246, 0302, 0326.
