IJ/SSQC Citation Update

There were a total of 54 Recertification surveys conducted by the Indiana State Department of Health in the month of February and of those, 3 were found to be deficiency free. There were a total of 29 G level federal tags cited in 21 locations throughout Indiana for F323 (12 times), F314 (7 times), F 309 (5 times), F325 (2 times), Tags F323, 327, and 520 were cited .

There were 3 facilities that received IJ/SSQC tags. One facility received F329 for failure to monitor a PT/INR ( international normalized ratio and laboratory blood clotting test) on two residents receiving physician ordered blood thinning medications. The first resident was admitted to the hospital with the diagnosis of severe Coumadin toxicity and significant anemia. Another resident required vitamin K treatment.

F223 was cited when the facility failed to ensure a resident was free from physical abuse by a staff member who smacked the resident on the face with a hand and with a pillow and used the pillow to cover the face of the resident . In another situation, F223 was cited for failure to ensure residents with dementia were not intimidated by staff who attempted to detour exit seeking behaviors by telling residents a "boogy man or bad man", "sick people" or a "hairy spider" were on the other side of the door.
